Please note you can fax forms to us for speed, but we will need the originals mailed to us, 90 days before your departure to Punta Arenas.

Required – We need a completed form in order to confirm your booking.

Required – By signing our terms and conditions you show us that you understand and accept our cancellation policies.

Personal Information »
Required – This form is important to us for preparation of passenger lists, for emergency contacts and insurance information.

Release of Liability/Waiver »
Required – When you sign our waiver you agree to the risks involved when traveling with us and, in the event of an accident, release us from liability and waive your right to sue.

Medical Form »
Required – This will be forwarded to our staff medics for review. We want to ensure that your chosen program/expedition is suited to your abilities and that we have complete information about any special needs or limitations you may have.

Clothing Hire »
For this program, you have the option to rent specialized Antarctic clothing and equipment from us. If you plan to use these items only once, it will be cheaper and more practical to rent from us rather than purchase.
